Závazná pravidla pro žádost o přijetí nového klienta do specializované péče NeuroCentrum Clinic // Binding rules for the application for admission of a new client to the specialized care of NeuroCentrum Clinic

Vzhledem k rostoucím požadavkům na naši péči je potřeba do objednávání vnést větší pořádek a ulevit tím mně a naší aktuálně přetížené recepci. Žádám všechny budoucí žadatele o naše služby o spolupráci, tedy aby se důsledně řídili níže uvedeným doporučením.

ŽÁDOSTI O PŘIJETÍ DO PÉČE NeuroCentrum Clinic posílejte e-mailem VÝHRADNĚ na adresu ambulance@neurocentrum.cz (recepci na adrese recepce@neurocentrum.cz nebo na telefonu +420 604 265 487 kontaktujte prosím jen tehdy, pokud jste již našimi klienty a potřebujete řešit naléhavou provozní záležitost, jako třeba potvrzení nebo změnu domluveného termínu, zaslání chybějící dokumentace apod.).

Žádost by měla obsahovat:

OSOBNÍ DATA DÍTĚTE

- jméno a příjmení dítěte

- rodné číslo dítěte

- zdravotní pojištění dítěte

KONTAKTNÍ ÚDAJE RODIČŮ (zákonných zástupců)

- korespondenční adresa

- telefon (uvádějte číslo/čísla, na které/která se vám můžeme kdykoliv dovolat, přijímejte prosím hovory a čtěte SMS z telefonního čísla naší recepce +420 604 265 487)

- e-mailovou adresu (kterou průběžně monitorujete nebo pravidelně kontrolujete, kontrolujte i složky jako "spam", "nevyžádaná pošta" apod.; odpověď Vám přijde z adresy zivny@neurocentrum.cz, ambulance@neurocentrum.cz nebo recepce@neurocentrum.cz)

- další komunikační kanály (WhatsApp, Messenger)

ANAMNÉZU

- podrobný popis předchorobí (těhotenství, porod, Apgar skóre, novorozenecká žloutenka a způsob její léčby, stav kyčelních kloubů a případná léčba, dosavadní psychomotorický vývoj, případná léčba, jiná souběžná onemocnění a jejich léčba, užívané léky a potravinové doplňky, alergie, prodělané operace, prodělané úrazy, dosavadní očkování a případné komplikace po něm)

- podrobný popis současného stavu dítěte (jeho aktuální dovednosti, vývojové odchylky, problémy, které u dítěte řešíte nebo o jejichž řešení usilujete, váš subjektivní názor na jejich příčiny a představy o způsobu řešení) a přehled dosavadní léčby současných problémů

- scany propuštecí zprávy dítěte z porodnice (porodopis), zpráv z dosud absolvování vyšetření (pediatrie, neurologie, ortopedie, rehabilitace, oční, ORL, ..., zobrazovací vyšetření, laboratorní vyšetření, genetika, ..., pokud jsou k dispozici)

- fotografie a videa dokumentující současný stav dítěte, případně i současné způsoby léčby (cvičení)

Uveďte prosím též informaci, kde jste se o nás dozvěděli (kupř. od poskytovatele zdravotní péče, od rodičů, kteří již mají dítě v naší péči, na iternetu - specifikujte kde, ze sdělovacích prostředků).

Připojte též explicitní SOUHLAS ze zpracováním a archivací zaslaných osobních údajů, poskytnutých informací a textového a obrazového materiálu pro potřeby vyšetření a poskytování zdravotní péče, odborné vědecké a publikační účely (v souladu s pravidly GDPR).

Takto zpracované žádosti budou mít vyšší pravděpodobnost přednostního uspokojení.

Před odesláním žádosti o přijetí do naší péče věnujte prosím pozornost také informaci o pravidle 1+1, které se týká omezení počtu doprovázejících osob.

- Detailed description of background (pregnancy, birth, Apgar score, neonatal jaundice and its treatment, hip condition and possible treatment, psychomotor development to date, possible treatment, other comorbidities and their treatment, medications and food supplements taken, allergies, operations, injuries, vaccinations to date and possible complications after vaccinations)

- a detailed description of the child's current condition (his/her current skills, developmental deviations, the problems you are addressing or trying to address, your subjective opinion of their causes and your ideas on how to address them) and a summary of the treatment of current problems to date

- scans of the child's discharge report from the hospital (obstetric history), reports of examinations carried out so far (paediatrics, neurology, orthopaedics, rehabilitation, ophthalmology, ENT, ..., imaging, laboratory tests, genetics, ..., if available)

- photographs and videos documenting the child's current condition, including current treatments (exercises, if applicable)

Please also indicate where you found out about us (e.g. from a health care provider, from parents who already have a child in our care, on the internet - specify where, from the media).

Please also attach an explicit CONSENT to the processing and archiving of the personal data sent, the information provided and the text and image material for the purposes of examination and provision of health care, professional scientific and publication purposes (in accordance with GDPR rules).

Requests processed in this way will have a higher likelihood of priority satisfaction.

Before submitting an application for admission to our care, please also pay attention to the information about the 1+1 rule, which refers to the limitation of the number of accompanying persons.
